Position: Case Manager
Location: Martinez, CA - fully on site
Hours: 4-10 schedule 6:00 AM - 4:30 PM
Temp ; possibly temp to hire
Family Self Sufficiency -Grant Compliance
Duties:
- Participate in quarterly FSS orientations in addition to updating PowerPoint with relevant information & setting up the orientation (Zoom & in-person when available)
- Complete monthly reconciliation & file audits
- Build partner relationships & creating more through partner outreach; maintain current partnerships
- Roundtable FSS applications for enrollment with FSS staff
- Complete FSS intake appointments (script, needs assessment, ITSP creation) & finalizing enrollment documents (file setup, type-8 50058, etc.)
- Complete case management appointments & follow-up via mail and email
- Update program forms, Excel VBA scripts, letters, etc.
- Update SOP/action plans/annual planning & track program outcomes
- Complete social media outreach via Facebook, including setting up and/or advertising workshops

Family Self Sufficiency -Administrative
Duties:
- Schedule RentCafe annual recertification appointments & mail letters to clients
- Receive and make interest calls, follow up via email and/or mail, and update Interest Card List
- Participate in quarterly FSS orientations and follow up by sending applications, collecting complete applications, and determining eligibility with checklist
- Roundtable FSS applications for enrollment with FSS staff
- Schedule and complete case management appointments & follow-up via mail and email
- Complete monthly case management filing and complete case management log audits every Thursday
- Assist with case management file audits
- Print, mail, and file documents as needed
